Interesting that you raise this problem.
On March 16, 2005, I filed a suggestion to have this
problem fixed in Abiword, and was told it really
wasn't a problem.
See:
http://bugzilla.abisource.com/show_bug.cgi?id=8645

FWIW, you might want to add to that thread a "me too."

(Note the suggestion was made over two years ago, when
v. 2.2.5 was current; since then, AbiWord has not been
modfied as to this point.)

In the meantime, one thing I found that a user can do
(which perhaps you already have tried and rejected as
unworkable for your purposes) is, by trial and error,
determine a combination of a top page margin and a
header size that can accomodate part of what you want
to appear at the top of page 1, and in that space put
as a header at the top of page 1 something such as the
company name or logo. The rest of what ordinarily you
would put in the header of page 1 put at the outset of
the body on page 1.
In the header space on page 2 and subsequent pages,
put what you would want to be a header on each of
those pages - if nothing else, a page number, the
current date, or the addressee's name and title.
I found that through trial and error, in large part
judging by the white space appearing between the
header and the body on each page, a reasonably
acceptable document could be achieved. Of course,
once you have found something that is acceptable, make
a template for it.
I didn't address the matter of the footer as it
appears from your last post on May 17 that you have
solved that problem.
