You need to save this template into the templates folder. On Windows
it's in Program Files\AbiSuite 2\templates, and I know the Mac templates
come in the App bundle, but a Mac dev can tell you better if you can
save them to anywhere else in order to make them work. Alternatively,
if you want to use a template only some of the time, the "Open as Copy"
will start a new, untitled document with the contents of the opened file
in it.

This sounds like a bug... please report this and any other bugs to
http://bugzilla.abisource.com However, I'm not a Mac user or dev, so
there may be some magic switch that was changed...

I presume you mean the line-selection arrow, that generally is an arrow
pointing right instead of left. If you don't click "just right" in the
header to edit it, you may select a line accidentally. Make sure you
have first inserted the header using Insert, Header. If you have
trouble getting the caret (insertion pointer) into the header, turn on
the "Extra" toolbar (View, Toolbars, Extra, or thereabouts) and click
the icon that has a purple rectangle at the top (or bottom for footers)
of the document.
