Some more work toward importing footnotes from word; I had to make a
change to pt_PieceTable::_tellAndMaybeAddListener() where I changed
bAddOffset on two occassions from false to true. I have no idea why
the original setting was false, because with it fragments following a
footnote (in a single block) were all getting the same block offset.
