Subject: other Normal View nitpicks

While I'm at it...
1. It looks like more of the page-centric drawing needs to be tweaked for
Normal View. For example:
  - the left margin is too big
  - the top ruler should probably be full-width, not page width
  - the page separator should be full-width, not just page width
  - there seems to be a one-pixel erasure problem immediately below it
  - forced page breaks should draw with a different full-width separator,
and should *not* leave the extra white space
  - there shouldn't be more than one screenful of white space beyond EOD
  - there should be a fat little underbar character for the EOD run
  - etc.
2. Likewise, there are more such problems in Show Paragraphs mode for
Normal View:
  - the crop marks should definitely *not* be drawn
  - the page / column borders should probably not be drawn
  - etc.
3. More generally, I got the impression that there might be an emerging
consensus that Normal View should be a single continuous vertical column
with *zero* page-oriented cues. Is this still the goal? We seem to be
heading in this direction on a tweak-by-tweak basis, but getting columns to
layout vertically instead of side-by-side seems like more than a tweak.
